Made In Chelsea star Sam Vanderpump has shared one of the most heartbreaking moments of his life after a doctor bluntly warned him that his rare liver disease is only going to get worse.
The emotional scene aired in the new E4 documentary Made in Chelsea: Sam Vanderpump’s Story, which follows the 28-year-old reality star as he battles end-stage liver disease while preparing for the possibility of a life-saving transplant.

Sam, who revealed last year that he may not survive more than five years without a transplant, was born with congenital hepatic fibrosis — a condition that remained dormant for years before suddenly becoming critical in late 2024.
During the documentary, viewers watched as doctors explained the shocking extent of the damage to his liver. One specialist told Sam that his liver was enlarged and filled with “big lakes of bile” caused by the abnormality he inherited at birth.

Trying to explain the illness in his own words, Sam said: “A normal liver is a smooth filter which filters people’s bloods. My liver isn’t smooth. It’s quite bumpy and the fibrosis and it’s got a cyst in it and essentially struggling to filter the blood.”
He added emotionally: “That disease has progressed over 28 years, which has obviously led to the inevitable.”
But the most devastating moment came when the doctor looked directly at Sam and admitted: “We’re not talking about if you will need a transplant, we’re talking about when you will need a transplant.”
The medic then delivered the crushing warning: “This is not going to get better, it’s going to get worse.”

Doctors also revealed Sam had already suffered three serious hospitalisations due to cholangitis infections, warning that another severe episode could quickly lead to organ failure.
The documentary follows Sam and his wife Alice Vanderpump as they attempt to navigate married life, parenthood and the terrifying uncertainty surrounding his health battle.
The timing makes the story even more emotional for viewers, as the couple only recently welcomed their first child together — a baby boy named Marmaduke Marcus Vanderpump.

The name carried deep emotional meaning for Sam, as it was originally loved by his late father Mark Vanderpump, who died in 2018.
Sam and Alice also rushed forward with their wedding plans after his terrifying hospitalisation with liver and kidney failure, which later developed into life-threatening sepsis.
The couple ultimately chose to marry in an intimate ceremony at Chelsea Registry Office because they wanted to share the same surname before their son arrived.
